Referrals

Sarah requires a referral from a General Practitioner (GP) prior to booking an appointment. A valid GP referral will enable clients to access Medicare rebates for eligible services. Referrals can be emailed to admin@drsarahbaker.com.

To facilitate assessment, referrals should include patient contact details, current concerns, relevant background information, previous mental health history, current medications, risk concerns and any previous psychiatric or psychological reports. Where available, copies of relevant school reports, previous psychological assessments and any educational or allied health reports are also helpful.

Please note: We do not provide emergency or crisis services. If you require urgent care, please contact your local area mental health service, call 000, or contact Lifeline on 13 11 14. In an emergency, you may also present to your nearest emergency department.
